Appointments and Cancellations
Please notify your clinician if you are going to be late for your appointment. For intake and follow up appointments, if you are more than 15 minutes late, your appointment will be rescheduled, and you will be charged a $150 fee. Regular attendance at scheduled appointments is a key component of successful treatment. If you cannot attend an appointment, you must cancel 48 hours/2 business days in advance to avoid being charged a cancellation fee.
Please note that Saturdays, Sundays and national holidays are not considered business days and notice of cancellation provided on those days will be deemed received on the next business day. If we do not receive notice of cancellation sufficiently in advance of your appointment, you will be responsible for the cancellation fee. This policy will be applied in all circumstances unless prohibited by law or our agreement with your insurer.
You are responsible for attending each appointment and agree to adhere to the following policy: If you cannot keep the scheduled appointment, you MUST notify our office to cancel or reschedule the appointment at least 48 hours/2 days prior to the scheduled appointment. The no show/cancellation fee is $150, which applies to no shows and cancellations without at least 48 hours/2 days notice prior to the scheduled appointment. We do not refund no show/cancellation fees. The first no-show/late cancellation may be waived at the discretion of Core Psychiatry. Multiple no shows or late cancellations within a 365 day period can result in discharge from the practice.
Payment Policy (Self-pay)Fees are due and payable upon completion of each visit with cash, check, or credit cardYou MUST have a valid credit card on file at all times regardless of how you wish to pay prior to any scheduled appointment.Please make checks payable to Core Psychiatry and include the patient's full name on the check. Overdue payments: An outstanding balance for more than 60 days will result in your account being placed on HOLD and your next appointment may be cancelled until payment is received. If your case is on HOLD longer than 30 days, you may be terminated from the practice.
